How to use this template

When you need it

Issue a work inspection request when a defined lot or work stage is ready for the inspection required by the project process. Assemble prerequisite records before requesting attendance, especially where subsequent work will conceal evidence.

What it helps you decide

It helps coordinate an inspection around a clear boundary and a credible readiness claim. Inspectors can identify the work, review supporting records and make an informed assessment without searching for missing context.

How to complete it

  1. Describe the lot, location and work identity, then reference the applicable ITP, checklist, current drawings and specification basis for the requested inspection.
  2. Record each prerequisite with its source, evidence reference and confirming person; attach relevant site records and photographs and disclose incomplete conditions clearly.
  3. State the requested inspection date and parties, submit through the controlled route, and retain the inspector assessment and authorized status with the package records.

Who prepares and reviews it

The contractor's site or quality engineer prepares the request with the work supervisor and relevant test personnel. The appointed inspector records assessment; transaction status and release decisions follow the project's actual delegation.

A mistake to avoid

An issued request proves that inspection was requested; it does not by itself prove attendance, acceptance or permission to cover the work.

Illustrative example

Illustrative example: a contractor requests inspection of a drainage trench before backfilling. The request identifies the chainage, pipe section and prerequisite records. The inspector finds that one test reference is missing, allowing the team to resolve readiness before arranging the affected inspection.
