Who prepares and reviews it
The inspector or quality engineer records evidence with the responsible discipline specialist. The contractor proposes disposition where appropriate; acceptance, technical concessions and closure belong to the roles authorized by the project arrangements.
A mistake to avoid
Do not combine the factual finding with an assumed cause or prescribe an unreviewed repair; each needs its own evidence and competent consideration.

Illustrative example: an inspection identifies installed pipe supports at locations inconsistent with the issued drawing. The NCR records measurements, photographs and source references. The responsible engineer evaluates a disposition, and closure awaits evidence that the accepted correction or other authorized resolution has been verified.
