How to use this template

When you need it

Use a material inspection request when an identifiable delivery lot arrives and the project's receiving process requires inspection before storage release or incorporation into the works. Prepare it using the actual delivery evidence.

What it helps you decide

It connects physical goods to their approved technical basis and certificates. The receiving team can decide whether identification, condition and documentation support acceptance, further checking or segregation pending disposition.

How to complete it

  1. Identify the delivery lot, manufacturer, quantity and lot, heat or serial references, checking them against delivery records and the approved material submittal.
  2. Attach certificate references and photographs, then record observed condition, storage arrangements and traceability gaps without assuming that a matching product name proves conformity.
  3. Submit the inspection request through the project route; retain the inspector assessment and authorized status, and connect any unresolved item to its controlled follow-up.

Who prepares and reviews it

The contractor's quality or materials engineer prepares the request with storekeeper and supplier records. The appointed inspector assesses the delivery; acceptance authority and any client participation depend on the project arrangements.

A mistake to avoid

Do not confuse a material submittal review with inspection of the delivered lot; substitution, damage or missing identification can arise after procurement.

Illustrative example: reinforcing bars arrive with several bundles carrying unreadable heat labels. The request records the affected quantities, photographs and available certificates. The inspector identifies the traceability gap, and the authorized party decides the segregation and verification route before those bundles enter fabrication.
