How to use this template

When you need it

Use this register when site observations identify hazardous conditions or ineffective controls and the team needs to track immediate action, corrective measures, verification, closure, and possible reopening through accountable follow-up.

What it helps you decide

It keeps the path from observation to verified control visible, preventing the administrative completion of an action from being mistaken for evidence that the underlying hazard or exposure has been adequately addressed.

How to complete it

  1. Record Observation ID, Date raised, Location / activity, and Observed hazard / condition with Observation evidence and a justified Priority / criteria basis.
  2. Document Immediate action, Control hierarchy, and Corrective action, assign Action owner and Due date, and keep Status consistent with the actual work completed.
  3. Complete verification fields using Completion evidence and Verification result, recording Closure date only after the responsible review and Remarks / reopening basis where further concern remains.

Who prepares and reviews it

Supervisors, workers, or HSE personnel may raise observations, while the register owner coordinates follow-up with action owners. Competent reviewers verify effectiveness, and project arrangements determine who may close significant findings or authorize further work.

A mistake to avoid

Closing an observation when a photograph is uploaded, without checking whether the control addresses the hazard, can leave the exposure unresolved despite a completed status.

Illustrative example

Illustrative example: an inspection finds an unprotected temporary floor opening near a delivery route. The register records immediate isolation, the corrective protection action, and supporting photographs. A competent verifier checks the installed control before closure, and the record remains available if later work removes the protection.
