Hold and Witness Point Register

Control inspection points, notice periods, responsible parties, evidence, attendance and release decisions by authorized personnel.

How to use this template

This Excel template contains prepared working sheets and completion notes on a separate sheet. Enter your project data and review the completed workbook before use.

This editable workbook helps you organise project information. Check any included formulas, assumptions and results before use; it does not replace professional review.

The form prepares a separate brief; it does not fill the Word or Excel file.

How to use this template

When you need it

Set up from approved inspection plans before controlled activities begin. Maintain it as notices, attendance responses and release records arrive, particularly before work conceals or passes beyond an inspection opportunity.

What it helps you decide

The register identifies the intervention points governing progression of work. It helps supervisors avoid proceeding on assumed release and makes unfulfilled notice or evidence requirements visible before they disrupt the programme.

How to complete it

  1. Identify Activity and point, Point type and Requirement source from approved inspection plans. Confirm the project's meanings of hold, witness and review interventions.
  2. Record Notice evidence against the actual recipient and verified timing basis. Link each point to its exact location and planned activity boundary.
  3. Enter Attendance or release evidence only from actual records. Retain an Unresolved restriction until the applicable conditions are fulfilled and documented by authorized personnel.

Who prepares and reviews it

The quality engineer maintains it with discipline inspectors, contractor planners and relevant client representatives. Attendance and release decisions belong to the parties authorized by the actual inspection plan and contractual arrangements.

A mistake to avoid

Assuming that a missed witness visit releases every related hold point ignores different intervention definitions and can permit work beyond an unresolved restriction.

Illustrative example

Illustrative example: reinforcement inspection is required before a foundation pour. The register links the applicable point and issued inspection notice, but release evidence is absent. The quality team obtains the responsible party's actual disposition before the superintendent decides whether the affected pour can proceed.
