How to use this template

When you need it

Use when instructed or disputed changed work requires a traceable proposed valuation. Apply after identifying baseline scope and actual valuation provisions, and update as measurements, rates and agreement status develop.

What it helps you decide

The worksheet connects the changed scope to quantity and price evidence. It exposes omissions, prior valuations and disputed components so a calculated amount is not confused with entitlement or an agreed price.

How to complete it

  1. Describe Change / item and Scope / quantity against original, omitted and added work. Link the instruction and flag work whose authority remains uncertain.
  2. Record Valuation provision, Rate basis and Supporting cost from actual terms and records. Enter independently checked amounts; this worksheet provides no automatic valuation or totals.
  3. Reconcile Omission / prior value before recording Assessed amount. Preserve Agreement status separately, distinguishing submitted, assessed, agreed and paid values and unresolved components.

Who prepares and reviews it

A quantity surveyor or commercial engineer prepares it with discipline and site measurement inputs. A commercial reviewer checks quantities and rate treatment; agreement or determination belongs to the parties authorized under the contract.

A mistake to avoid

Pricing all added work while leaving the superseded scope in previous valuations can overstate the proposed change and duplicate recovery across payment submissions.

Illustrative example

Illustrative example: a revised drainage alignment adds pipework but removes part of the original route. The worksheet links measured changes and applicable rate evidence, including the omitted portion. The commercial reviewer checks the net proposal before seeking agreement, without treating the arithmetic as proof of contractual entitlement.
