How to use this template

When you need it

Use this document when the role empowered by the contract needs to instruct a defined change to the works or services, whether its price and time effects are settled or pending.

What it helps you decide

It establishes exactly what is instructed, which documents govern execution, and how unresolved valuation or programme matters are treated, reducing ambiguity between technical direction and commercial agreement for delivery teams.

How to complete it

  1. Verify Contract and authority against the executed agreement, identifying the actual provision and role permitting the proposed instruction before issue occurs.
  2. Define Change to the works or services through specific revised documents, locations, additions, deletions, boundaries, and interfaces with work already underway.
  3. Record Price and time treatment separately, attach applicable conditions, and obtain authorized issue and acknowledgement without implying agreement from receipt alone.

Who prepares and reviews it

An authorized contract administrator usually prepares or issues the instruction with design, commercial, and planning advice. The contractor acknowledges receipt; approval and signature requirements follow the actual contract's allocation of authority.

A mistake to avoid

Using one signature to imply approval of scope, price, and time obscures unresolved matters and may misrepresent the recipient's actual acknowledgement.

Illustrative example

Illustrative example: an engineer instructs a revised drainage route around an existing foundation. The record identifies the new drawing revision and affected chainages, authorizes the defined work, and marks valuation pending. The contractor acknowledges receipt while submitting a separate assessment of cost and programme effects.
