Test Report and Laboratory Results Register

Register laboratory and site test results, acceptance criteria, equipment, status and follow-up using actual controlled test reports.

How to use this template

This Excel template contains prepared working sheets and completion notes on a separate sheet. Enter your project data and review the completed workbook before use.

This editable workbook helps you organise project information. Check any included formulas, assumptions and results before use; it does not replace professional review.

The form prepares a separate brief; it does not fill the Word or Excel file.

How to use this template

When you need it

Maintain as laboratory and site reports arrive during construction or commissioning. Use it when numerous samples, lots and test requests must be reconciled before work acceptance or document handover.

What it helps you decide

The register makes missing, qualified or unresolved results visible without replacing laboratory evidence. It helps reviewers locate the original report and distinguish the reported measurement from the project's acceptance decision.

How to complete it

  1. Register Report number and Original report reference exactly as issued. Check the laboratory, report revision and relevant capability or accreditation scope where project requirements demand it.
  2. Match Sample or asset and Method and date to the originating sample and test request. Resolve identifier mismatches before relying on the result.
  3. Record Reviewer disposition separately from the reported result. Use Missing evidence to track absent reports, qualifications and requested retests until supported records resolve them.

Who prepares and reviews it

The quality records coordinator usually maintains it using laboratory reports and sample records. Discipline engineers review technical implications; the person authorized under the inspection arrangements makes and records the applicable disposition.

A mistake to avoid

Copying a reported value into an acceptance status without checking the sample location and criteria confuses measurement evidence with the responsible engineer's disposition.

Illustrative example

Illustrative example: a concrete package contains reports for most pours, but one sample identifier does not match the site sampling log. Record the report as received with traceability unresolved. The quality engineer seeks laboratory and sampling clarification before deciding whether the result can support the affected pour's review.
