How to use this template

When you need it

Use this request when a proposed or existing departure from a specified technical requirement needs a controlled decision, including cases where a nonconforming item might be accepted only within defined conditions or limits.

What it helps you decide

It makes the requirement, proposed difference, evidence, alternatives, and implications explicit, allowing competent authorities to assess the departure without confusing commercial urgency, technical acceptability, and authorization to implement or retain the item.

How to complete it

  1. Identify Affected item and request scope and state Requirement and proposed departure precisely, distinguishing a planned deviation from an existing nonconformity and its affected quantities.
  2. Explain Reason, alternatives and supporting evidence, then assess Technical and project implications using relevant calculations, records, and specialist input actually available for the decision.
  3. Record Required reviews and decision with explicit status, then define Implementation limits and verification, referencing any separately authorized design or commercial changes needed for implementation.

Who prepares and reviews it

The responsible engineer or contractor typically prepares the request with design, quality, construction, operations, and supplier input. Competent technical authorities assess acceptability, while client and commercial roles decide matters within their actual approval responsibilities.

A mistake to avoid

Treating an approved commercial substitution as technical acceptance can bypass the competent review needed to assess performance, durability, safety, and operating implications of the departure.

Illustrative example

Illustrative example: delivered pipe supports differ from the specified protective finish. The request identifies the affected tags, requirement source, proposed departure, and evidence on exposure conditions. The technical reviewer rejects unsupported acceptance and asks for a compliant alternative or substantiated assessment before the supports can be retained.
