How to use this template

When you need it

Use this assessment when a project team needs an evidence-based review of collective effectiveness, particularly after delivery difficulties, organizational changes, or a completed phase that offers clear opportunities for improving collaboration.

What it helps you decide

It separates observed team performance from personal impressions and converts findings into practical improvement commitments, helping the group address coordination, decision-making, and delivery habits that influence its ability to achieve shared objectives.

How to complete it

  1. Define Assessment purpose and evidence using the relevant period, shared objectives, delivery records, and feedback that can substantiate observations about the team's work.
  2. Complete Team effectiveness review by distinguishing recurring coordination patterns from isolated events, and discuss alternative explanations before deciding where improvement is needed.
  3. Agree Improvement commitments with responsible participants, then define Follow-up and feedback so subsequent assessment checks whether working behaviour and delivery outcomes actually improve.

Who prepares and reviews it

The team leader usually facilitates preparation with team members and relevant interface partners. A project manager or agreed reviewer considers the evidence, while personnel decisions remain subject to the organization's actual management and confidentiality arrangements.

A mistake to avoid

Ranking individuals from a team-level assessment can suppress useful feedback and distract from shared processes or interfaces that actually explain the delivery problem.

Illustrative example

Illustrative example: a design team repeatedly misses coordinated issue dates despite completing individual drawings. The assessment reviews interface records and finds that unresolved cross-discipline decisions arrive too late. The team commits to earlier interface reviews, and the next assessment checks whether coordinated releases become more reliable.
