How to use this template

When you need it

Use this register during fieldwork and evidence consolidation whenever measurements or photographs support an asset assessment. Establish identifiers early so observations from multiple inspectors can be connected to the same element and location.

What it helps you decide

It makes evidence interpretable after the site visit. Reviewers can understand where a record came from, how a measurement was obtained and what reliability limits affect its use in an assessment.

How to complete it

  1. Assign record identifiers to original photographs and measurement notes, mapping each to the asset and location using consistent element references from the survey scope.
  2. Record values and units where measured, together with the method or instrument and contextual reliability notes needed to understand accuracy, visibility or operating conditions.
  3. Check that references locate the original files, preserve provenance and retention arrangements, and link the records to survey findings without replacing originals with unexplained extracts.

Who prepares and reviews it

Inspectors and instrument operators provide original records and method details. The survey coordinator controls references, while the relevant technical assessor checks suitability of the evidence for the intended engineering conclusions.

A mistake to avoid

Do not rely on camera timestamps or filenames alone for location; inconsistent device settings and copied files can obscure the record's true context.

Illustrative example

Illustrative example: two surveyors photograph the same corroded connection from different levels. The register ties both images and a thickness measurement to the identified asset location. The assessor can compare evidence correctly and decide whether further measurement is needed to understand the observed condition.
