How to use this template

When you need it

Use this review when a supplier or subcontractor's delivery, quality, documentation, and response performance needs a documented assessment, particularly before improvement discussions, future procurement decisions, or escalation of recurring package problems.

What it helps you decide

It grounds procurement recommendations in comparable evidence and supplier explanations, helping the project distinguish isolated events from recurring performance patterns and agree practical improvements without treating an assessment as a unilateral contract change.

How to complete it

  1. Define Supplier package and review basis using the actual order, period, and agreed measures, then assess Delivery and quality performance with consistent units and denominators.
  2. Document Significant events and impacts, distinguishing observed effects from disputed causation, and record Review discussion and supplier comments with supporting evidence or factual corrections.
  3. Agree Improvement actions and follow-up, then record Procurement recommendation and decision through the authorized role, leaving unverified results open until the relevant evidence is available.

Who prepares and reviews it

The procurement or package manager typically prepares the review with quality, engineering, logistics, and commercial contributors. The supplier responds to findings, while the authorized procurement authority makes future-use or escalation decisions under the actual governance process.

A mistake to avoid

Averaging unrelated delivery and quality percentages without a documented method can create a favourable overall score that hides a serious weakness in a critical requirement.

Illustrative example

Illustrative example: a switchgear supplier delivers equipment on time but repeatedly submits incomplete test records. The review distinguishes physical delivery from documentation performance and records the supplier's explanation. The parties agree a documentation action, and procurement reviews verified improvement before deciding the supplier's suitability for another package.
