How to use this template

When you need it

Start the risk register during project definition and maintain it as design, procurement and site information develops. Add uncertain future events when their causes and potential effects can influence project objectives or decisions.

What it helps you decide

It converts vague concerns into owned response decisions. The team can compare exposure using agreed criteria, monitor triggers and judge whether proposed actions reduce uncertainty or only change its description.

How to complete it

  1. Confirm the approved risk criteria and describe each risk as a cause, uncertain event and consequence supported by the available project evidence.
  2. Assign category, owner, likelihood and impact using those criteria; record responses, action owners and due dates with evidence supporting the initial assessment.
  3. Review triggers and response progress, reassess residual ratings on a justified basis, and escalate or close entries only through the defined status authority.

Who prepares and reviews it

The risk coordinator facilitates contributions from package owners and specialist advisers. Risk owners accept responsibility for responses, while management confirms escalation and tolerance according to the project's agreed governance.

A mistake to avoid

Do not replace evidence with color ratings, or classify an already occurring problem solely as a risk; current issues also need active resolution.

Illustrative example

Illustrative example: incomplete utility records create uncertainty about buried obstructions along a cable route. The register records possible effects and an investigation response. After trial-pit evidence becomes available, the owner reassesses exposure and management decides whether the route or contingency approach needs adjustment.
