How to use this template

When you need it

Use this report when an independent or sufficiently objective review is needed to assess how the project's risk process and selected responses operate against stated criteria, beyond routine register maintenance or status reporting.

What it helps you decide

It distinguishes process compliance from response effectiveness, allowing leadership to see whether risk arrangements influence actual decisions and whether recurring weaknesses require corrective action, better evidence, or changes to project controls.

How to complete it

  1. Define Audit scope, criteria and independence, then perform Risk process review using actual identification, assessment, escalation, and monitoring records within the agreed sample.
  2. Assess Risk response effectiveness and Patterns and overall assessment, separating observed evidence from limitations or assumptions that the audit could not verify directly.
  3. Record Corrective and improvement actions and Audit conclusion and follow-up, assigning practical verification needs without implying assurance beyond the scope and evidence actually examined.

Who prepares and reviews it

A suitably independent auditor or review lead normally prepares the report with evidence from risk owners and controls teams. Relevant specialists assess technical responses, while management assigns corrective-action ownership and reviews follow-up within the agreed audit mandate.

A mistake to avoid

Concluding that risk management is effective merely because the register is regularly updated ignores whether responses work and important risks influence actual project decisions.

Illustrative example

Illustrative example: an audit samples major procurement risks on a power project. It finds current ratings but no evidence that overdue mitigation actions reached the decision forum. The report identifies the process weakness, assigns a corrective action, and specifies follow-up evidence showing that escalation works.
