Revision and Supersession Register

Control active/superseded versions, change description, authority, effective date and downstream impact.

How to use this template

This Excel template contains prepared working sheets and completion notes on a separate sheet. Enter your project data and review the completed workbook before use.

This editable workbook helps you organise project information. Check any included formulas, assumptions and results before use; it does not replace professional review.

The form prepares a separate brief; it does not fill the Word or Excel file.

How to use this template

When you need it

Use this register when a controlled document revision replaces another or when a change affects distributed copies. Maintain it through delivery so workfronts, reviewers and archives retain a clear relationship between current and historical information.

What it helps you decide

It prevents a new upload from silently obscuring the replacement process. The team can identify who needs the change, what prior issue is superseded and where the historical evidence remains available.

How to complete it

  1. Identify the document and its current register entry, then verify the authorized revision or change record establishing the relationship between superseded and replacement issues.
  2. Record old and current revisions, change authorization and required distribution action, checking which dependent packages or recipients need the updated information for their work.
  3. Retain archive references and evidence of distribution, resolve inconsistent copies and confirm the active revision through the controlled process rather than deleting the previous issue.

Who prepares and reviews it

Document control maintains revision chains with originators and discipline owners. Technical owners confirm authorized change scope; the designated issue authority governs effective status, distribution and retention under the project's document procedures.

A mistake to avoid

Do not erase superseded records to simplify folders; historical versions may be necessary to explain instructions, reviews and work completed before the change.

Illustrative example

Illustrative example: a revised equipment foundation detail changes anchor locations. The register links the replacement drawing to the prior revision and change authorization. Document control coordinates distribution to fabrication and site teams, allowing them to assess whether work based on the earlier issue needs intervention.
