How to use this template

When you need it

Use as failures, degraded operation and restoration events occur, before calculating reliability measures. Apply consistent definitions across assets and periods so later trends are based on comparable event records.

What it helps you decide

The register separates equipment repair from service restoration and records the time basis behind downtime. It exposes duplicate events, uncertain timestamps and denominator gaps that can distort reliability or availability reporting.

How to complete it

  1. Define Event / asset and Operating state using the agreed event rules. Distinguish failure, planned outage and degraded service within the actual asset hierarchy.
  2. Record Onset / detection and Repair / restoration from source logs. Explain Downtime category, retaining waiting, access and parts delays separately where evidence allows.
  3. Check Cause status and Source / quality, then confirm Metric inclusion. Review operating exposure and duplicate events before a separate analysis calculates any reliability measure.

Who prepares and reviews it

Operations or maintenance data staff maintain entries with event owners and technicians. A reliability specialist reviews definitions and exclusions; the asset owner approves reporting conventions and actions arising from the analysis.

A mistake to avoid

Using calendar hours for one asset and operating hours for another without clear definitions makes apparently comparable reliability figures describe different exposures.

Illustrative example

Illustrative example: a pump is mechanically repaired in the morning, but the system returns to service later after operational checks. The register retains both timestamps and the waiting category. A reliability analyst can then calculate the appropriate measure without treating repair completion as restored service.
