How to use this template

When you need it

Use this report at a project decision or review point when leadership needs an integrated view of significant threats, opportunities, response performance, emerging interactions, and the decisions required from management.

What it helps you decide

It turns detailed risk records into an evidence-based management view, emphasizing the overall position and response effectiveness so leadership can allocate attention and resources to uncertainty that matters to project objectives.

How to complete it

  1. Set Reporting boundary and overall position using the current review period, project scope, and source records, making significant data limitations visible from the outset.
  2. Assess Significant risks and opportunities and Response effectiveness, then explain Emerging issues and interactions rather than presenting risk ratings without their changing context.
  3. State Priority decisions and resources, identify responsible decision routes, and complete Data limitations and next review so unresolved evidence gaps remain visible to leadership.

Who prepares and reviews it

The risk manager or project controls lead usually prepares the report with risk owners, planners, estimators, and technical contributors. Project leadership reviews priorities, while the designated authority decides escalated responses and resource commitments.

A mistake to avoid

Copying the full risk register into a report without explaining changes, interactions, and decisions needed hides the management issues beneath an undifferentiated list.

Illustrative example

Illustrative example: a process plant reports that delayed vendor information now threatens both foundation design and equipment procurement. The report explains this interaction, reviews the response already attempted, and requests a client decision on an alternative supplier. Management can assess one coordinated action rather than two disconnected risks.
