How to use this template
When you need it
Use this register when key project outcomes and decision points need a controlled record of their required conditions, baseline commitments, current forecasts, actual achievement, ownership, and supporting evidence across the project.
What it helps you decide
It separates a milestone's date from the condition that makes it achieved, helping managers track genuine delivery outcomes and explain movement without confusing a forecast, a target, and an evidenced completion event.
How to complete it
Record Milestone ID, Milestone / outcome, and Required condition, then identify Date basis / source so each milestone has an understandable purpose and authority.
Keep Baseline date, Forecast date, and Actual date distinct, assigning Owner and Status based on the current evidence rather than the desired reporting position.
Link Evidence / decision to the achievement or authorized change, and reconcile significant forecast movement with the relevant programme and management decision records for review.
Who prepares and reviews it
The project planner or controls lead usually maintains the register with package owners, client representatives, and commercial contributors. Responsible authorities confirm achievement and approved baseline changes according to the project's actual governance or contractual requirements.
A mistake to avoid
Entering an actual milestone date because the planned date has arrived turns a tracking register into a record of intentions rather than evidenced achievement.
Illustrative example
Illustrative example: a plant programme shows the milestone ready for energization. The register defines the required readiness condition and separates the baseline date from the latest forecast. Because one prerequisite remains unresolved, the actual date stays blank even though physical installation is substantially complete.
