How to use this template

When you need it

Prepare after an actual meeting using attendance and discussion records. Circulate through the project process while details remain clear, especially where decisions, assigned actions or unresolved technical matters need reliable follow-up.

What it helps you decide

Minutes preserve what was discussed and what was actually decided. They make action ownership traceable while avoiding the conversion of suggestions or informal discussion into contractual instructions or approvals.

How to complete it

  1. Use Item and Topic and discussion summary to follow the agenda. Record Decision or unresolved point separately, with exact References supporting important statements.
  2. In Actions, assign stable identifiers, actual owners and agreed dates. Leave unconfirmed responsibility or timing explicit rather than supplying convenient assumptions after the meeting.
  3. Record Open matters and Next meeting and circulation using actual attendance. Follow the correction process without implying that circulation or silence automatically establishes acceptance.

Who prepares and reviews it

The meeting recorder prepares the minutes with clarification from topic leads. The chair or appointed reviewer checks accuracy; participants provide corrections under the agreed process, and decision authority remains with the relevant project roles.

A mistake to avoid

Writing a discussed proposal as an agreed instruction can send the site team into changed work that the responsible designer or contractual authority has not authorized.

Illustrative example

Illustrative example: a coordination meeting discusses moving a pipe route, but the designer requests another check before agreeing. The minutes record the proposal as unresolved and assign the check to its owner. The site team can then distinguish a pending design question from an authorized change.
