How to use this template

When you need it

Use for one defined maintenance job from request and planning through actual completion. Maintain it when findings change the work boundary, evidence remains outstanding or return to service requires a separate decision.

What it helps you decide

The work order connects authorized scope with actual work, materials and verification. It lets supervisors distinguish planned tasks, performed actions and open defects without treating completion reporting as permission to operate.

How to complete it

  1. Complete Work order identity with asset, task reference and evidenced priority. Define Authorized scope and technical instruction using the exact approved procedure and boundaries for escalation.
  2. Check Readiness and work authorization references for competence, parts, tools, access and applicable permits. Record issued authorizations separately; the work order itself establishes no isolation.
  3. Record Actual work and findings and Resources and materials used from performed work. Complete Completion, verification and return to service with actual evidence and unresolved defects.

Who prepares and reviews it

A maintenance planner prepares the order with the responsible supervisor and asset engineer. Technicians record actual work; competent verifiers assess results, and the operator's designated role makes the separate return-to-service decision.

A mistake to avoid

Closing the order because the technician finished the task can hide outstanding verification or defects and wrongly imply that the equipment has been authorized for service.

Illustrative example

Illustrative example: a work order calls for replacement of a leaking valve under an approved maintenance procedure. The technician records the installed part and an additional observed defect outside scope. The supervisor escalates that defect, while the designated operator separately reviews verification evidence before authorizing return to service.
