How to use this template

When you need it

Open an issue entry when a current problem requires resolution, coordination or a management decision. Use it during any project phase to keep active impediments visible until the agreed resolution is evidenced.

What it helps you decide

It distinguishes problems already affecting delivery from uncertain future risks. The team can focus decisions on affected scope, accountable ownership and a clear closure condition rather than repeatedly restating symptoms.

How to complete it

  1. Describe the current problem and supporting records, identify affected scope, and separate established facts from assumptions still requiring investigation by the responsible team.
  2. State the decision needed and owner role, then develop resolution actions using relevant schedule, cost or interface information without treating the register as approval.
  3. Review progress through the escalation arrangements and attach closure evidence showing that the issue's agreed resolution condition has actually been met before retiring it.

Who prepares and reviews it

The project coordinator or controls lead maintains entries supplied by workstream owners. Relevant technical or commercial specialists assess implications; the responsible manager determines escalation and closure within the project authority arrangements.

A mistake to avoid

Do not close an issue simply because an action was assigned; assignment starts resolution, while closure requires evidence that the problem has been addressed.

Illustrative example

Illustrative example: an issued design package lacks a required equipment interface dimension. The issue register records the affected foundation work and the decision needed from the designer. A confirmed interface drawing later supports the decision to close the issue and update the workfront plan.
