How to use this template

When you need it

Use this assessment when one risk needs more analysis than a register entry can provide, particularly where response choices, triggers, contingency arrangements, or residual exposure require a documented management decision.

What it helps you decide

It connects the risk scenario and evidence to practical response options, allowing reviewers to understand what could happen, why the selected response is proportionate, and how effectiveness will be checked.

How to complete it

  1. Define Risk identity and scenario with cause, uncertain event, and consequence, then complete Evidence and assessment using the approved criteria and available records.
  2. Compare Response options and selection, documenting practical constraints and expected benefits, then establish Triggers and contingency appropriate to the specific scenario under review.
  3. Assign Actions and effectiveness checks, reassess Residual position and closure, and retain unresolved assumptions until evidence supports a changed rating or closure decision.

Who prepares and reviews it

The risk owner usually prepares the assessment with relevant engineers, planners, cost specialists, and delivery representatives. A risk facilitator supports consistency, while the authority responsible for the affected objectives approves significant response or contingency decisions.

A mistake to avoid

Closing a risk because its action is completed confuses response implementation with proof that the underlying uncertainty or residual exposure has actually disappeared.

Illustrative example

Illustrative example: a bridge project may lose river access during seasonal high water. The assessment compares advance material positioning with an alternative delivery route and records the evidence behind each option. Management selects preparatory storage and defines the observation that would trigger the alternative route.
