How to use this template

When you need it

Use after a significant or recurring equipment failure when the underlying mechanism is uncertain. Begin while logs, damaged components and witness accounts can still be preserved for competent examination.

What it helps you decide

The review tests alternative explanations against evidence instead of stopping at symptoms. It connects corrective action to supported causes and defines how effectiveness will be checked after the repair is complete.

How to complete it

  1. Define lost function and the actual sequence from operating logs. Preserve inspection and component evidence, identifying changed or missing records and relevant custody limitations.
  2. For each Hypothesis / finding, record Supporting evidence, Contradictory evidence and Missing test / record. Keep Cause status provisional where alternatives remain credible.
  3. Link Corrective action to supported causes and define Effectiveness check. The Responsible reviewer distinguishes implementation, successful verification and final closure rather than merging them.

Who prepares and reviews it

A competent investigation lead coordinates operations, maintenance and discipline specialists. Evidence custodians support traceability; an independent reviewer challenges hypotheses, and the asset owner authorizes corrective actions and closure under the actual process.

A mistake to avoid

Naming operator error as the root cause without testing equipment, procedure and organizational evidence can produce an ineffective action and leave the failure mechanism intact.

Illustrative example

Illustrative example: a pump bearing fails repeatedly after replacement. The investigation compares alignment records, operating logs and preserved components against alternative explanations. Evidence may justify a targeted corrective action, but the review remains open until subsequent operating evidence demonstrates that the relevant mechanism has been addressed.
