Design Review Comment Sheet and Register

Capture each review comment, source, classification, response, owner, due date and disposition by an authorized reviewer.

How to use this template

This Excel template contains prepared working sheets and completion notes on a separate sheet. Enter your project data and review the completed workbook before use.

This editable workbook helps you organise project information. Check any included formulas, assumptions and results before use; it does not replace professional review.

The form prepares a separate brief; it does not fill the Word or Excel file.

How to use this template

When you need it

Use this register during formal design reviews and subsequent response cycles to preserve individual comments. Maintain it until the authorized reviewer has assessed the designer's response and the evidence supporting each proposed disposition.

What it helps you decide

It provides continuity between a technical concern, the response and the revised design. The team can distinguish answered comments from resolved comments and identify recurring issues across submissions or disciplines.

How to complete it

  1. Identify the reviewed documents, revisions and review remit, then assign comment identities with precise locations and a technical basis tied to the adopted criteria.
  2. Record the designer's response and relevant revision evidence, retaining the original comment so later reviewers can understand the issue and evaluate whether it was addressed.
  3. Assess responses against the technical basis, record the authorized disposition and carry unresolved items into the next review cycle with their supporting references intact.

Who prepares and reviews it

Reviewers raise comments with source and locator details, while designers provide responses and revision evidence. The review coordinator maintains the register; only the designated reviewer determines disposition under the project arrangements.

A mistake to avoid

Do not delete comments after a response arrives; the response, supporting revision and reviewer disposition are all needed to explain the closure decision.

Illustrative example

Illustrative example: a reviewer questions equipment removal clearance on a layout. The designer answers that access is adequate, but provides no revised detail. The register retains the open disposition until evidence allows the authorized reviewer to determine whether the concern has actually been resolved.
