It preserves why a choice was made and what must follow. Later teams can distinguish a binding project decision from a recommendation, discussion point or option that was never selected.
The project coordinator records decisions using meeting or formal approval evidence. Technical and commercial contributors confirm context; the delegated decision maker validates the actual choice and conditions within their authority.
Avoid converting meeting consensus into formal approval unless the decision maker had the necessary delegation and the recorded decision satisfies the applicable process.
Illustrative example: a project chooses modular electrical rooms after evaluating access and delivery options. The log references the authorized decision and its transport-study condition. Procurement can see the chosen direction while recognizing that an unresolved condition still requires evidence before the next commitment.
