Commissioning Test Sheet

Record the approved test steps, actual test results and supporting evidence. The responsible test engineer signs the results; the authorized approver records release.

How to use this template

This Excel template contains prepared working sheets and completion notes on a separate sheet. Enter your project data and review the completed workbook before use.

This editable workbook helps you organise project information. Check any included formulas, assumptions and results before use; it does not replace professional review.

The form prepares a separate brief; it does not fill the Word or Excel file.

How to use this template

When you need it

Prepare the test sheet from an approved commissioning procedure before carrying out the defined test. Use it during execution to record actual configuration, observations and measurements, then retain it for technical review and disposition.

What it helps you decide

It preserves what actually happened against the verified acceptance basis. Reviewers can distinguish measured performance from assumptions and decide whether the result supports acceptance, investigation or a controlled repeat test.

How to complete it

  1. Identify the test and system configuration, confirm procedure and acceptance sources, and record instrument details and prerequisites needed to interpret the results correctly.
  2. Capture each test step's actual measured value, unit and instrument with the observation reference; preserve exceptions or unexpected behavior instead of adjusting records to expected values.
  3. Compare results with the verified source through the competent review process, record the reviewer disposition and retain supporting evidence for any further test or release decision.

Who prepares and reviews it

The test engineer records results with instrument operators, vendors and designated witnesses as relevant. The responsible engineer signs the observations; acceptance or release is recorded by the project's authorized approver.

A mistake to avoid

Do not prefill measured values from design data; expected performance and actual observations serve different purposes and must remain distinguishable in the record.

Illustrative example

Illustrative example: a pump test records flow and pressure at the operating conditions specified by the approved procedure. An unstable reading is preserved with its instrument reference. The engineer evaluates the anomaly before deciding whether more investigation or a repeat measurement is needed.
