How to use this template

When you need it

Prepare a change order when the project's contractual process requires a controlled record of an authorized change, including the agreed or unresolved effects on the scope, price, and time.

What it helps you decide

It consolidates the change's boundaries and commercial status into a traceable contract record, helping delivery and finance teams implement the same decision without losing conditions, exclusions, or pending assessments.

How to complete it

  1. Confirm Contract and authority, related request, and the actual change mechanism before drafting the additions, deletions, and revised documents being ordered.
  2. Reconcile Price and time treatment with supporting assessments, stating currency, tax basis, effective date, and any valuation or extension still pending.
  3. List conditions and attachments, then complete Authorized issue and acknowledgement using the contract's roles, preserving the distinction between receipt and agreement.

Who prepares and reviews it

A commercial or contract manager typically drafts the order with technical and planning contributors. Signatories must hold the required contractual authority, and finance or client reviewers participate under project-specific approval arrangements.

A mistake to avoid

Copying a quotation total into the order without reconciling exclusions and pending programme effects can create a misleading record of agreement.

Illustrative example

Illustrative example: a utility building gains an additional transformer bay. The change order references the accepted technical revision and negotiated pricing attachment, records the agreed completion adjustment, and excludes a separately assessed utility fee. Authorized representatives sign the defined change for implementation and budget control.
